The overarching aim of the Service is to help to reduce inappropriate admissions to hospital or premature admission to long term residential care, by providing a high-quality service delivered by staff that are appropriately qualified and trained to enable people with Dementia to live independently as long as possible.

The Contract will be for a term of two years with the opportunity to extend for one year and a further one year subject to mutual agreement between the successful Bidder and the Authority. The maximum term of Contract is consequently four years.

The available budget is £245,759 per annum. This figure includes all aspects of the service to be delivered, any bidder including caveats to this service may be considered as submitting a non-compliant bid. The value stated in this notice is for a four year duration.
